I put my name and birthday on top, and then I start writing like I'm writing an uncle. I ask him to grant access and which spirits I need to work with and why. I told him the truth about my condition and what I want it to be in the future, ask for his guidance.
You can also google petition and how you want it to be. I just wrote a heartfelt letter basically.
