So I am getting ready to purchase the sugarcoat ritual from Mama Tina anddddd I am a little confused on how to fill out the form (I searched the forum before asking LOL). The first name should be mine. Then it says Full Name , DOB and Target Name.

Soo...after the email is it

my name, my dob and his name?

Or

my name, his dob and his name?
